📧Required Forms
Several national TSA forms, including the Code of Conduct form and the Authorization form, are mandatory for conference attendance. The forms require attendee/parent/guardian signatures and must be collected by chapter advisors for each person (members, advisors, and guests) listed on the chapter’s conference invoice. If requested, chapter advisors must provide the completed forms onsite at the conference to a state advisor or national TSA representative.
Other forms – from the local, district, or state level – may also be required for travel/hotel stay (including the required form needed to obtain tax exemption). Be sure to inquire about, secure, and complete such forms.
Also of note, for those traveling to the conference by air: As of May 7, 2025, everyone (18 years and older) must be “real ID compliant” to travel through airport security for flights in the U.S. Check for details/acceptable forms of ID (established by Homeland Security).
